Creating/Editing a Customized Form Page
The Website Manager allows you to create a Customized Form page (such as Survey Form, Polling Form, Registration Form, etc.) for your website:
1.Login to your Control Panel. 1
2.Proceed to the Website Designer Admin Area. 2
3.Select the Form Page radio button in the Choose the type of webpage page, and click Submit.

5.To add a Readymade Feedback Form to your website, click the GO button below the Simple Form and this will load the following page -

6.This page allows you to start providing the details of the customized form:
Title for Form Page: The title that you enter here will be displayed above the Form on the final webpage.
Text message to be displayed below Title: This message will be displayed above the Form on the final webpage, just below the Title. You may include HTML Content, Links and Images through a rich text editor, that will be presented after clicking the Advanced Text, Link & Image Insertion >> link.
Various fields on the Form:
1.Field Name: This is the name of the field on the final form.
2.Field Type: This could be a Text Field (Single Line), Text Area (Multi Line), Check Box, Radio Button, or a Pull Down Menu.
3.No of Options: This denotes the number of check boxes or radio buttons or number of entries in the pull down menu that is required by you.
4.Make field Mandatory: You can also make your visitors to compulsorily fill up a particular field, by clicking the appropriate checkbox under the this column.
Text message to be displayed below the form: This text will be displayed on top the Feedback Form on your website. You may include HTML Content, Links and Images through a rich text editor, that will be presented after clicking the Advanced Text, Link & Image Insertion >> link.
The visitor inputs will be sent to this email address (This will not be shown to the visitor): The email address specified under the Change the Title, Slogan & Email Address section under general Website Manager settings is pre-filled in this field. You may add additional email addresses separated by commas.

Message to be displayed after the Form is successfully submitted by the visitor: This message would be displayed to the visitor once he submits the details through the Feedback Form. You may include HTML Content, Links and Images through a rich text editor, that will be presented after clicking the Advanced Text, Link & Image Insertion >> link.
7.Once you submit all the required information for your Form, you can then proceed to provide the individual values for each of the field category options that you had created.

8.Once you provide all the values for the individual field options, you can then proceed to preview your customized form as it would appear on your website by clicking the PREVIEW button.
9.Click APPROVE to add the form to your website.
